This talk will provide a high-level overview of the MHRA, EMA and FDA options, including fast track programmes and the Innovative Licensing and Access Pathway (ILAP).

 In the rare disease space, specific considerations about trial design and study execution need to be carefully taken into account in order to successfully deliver results in a reasonable time frame. This talk will cover which aspects of clinical development are unique to rare diseases.

Pharmacogenetics can be defined as the study and application of genetics to optimise drug response.  In clinical practice, there is notable interindividual variation in drug response and recent trial evidence has demonstrated the value of implementing genetic testing relevant to multiple licensed drugs.  In drug discovery and development, genetics is being leveraged to identify novel drug targets, discover candidate biomarkers, and refine and optimise participant selection into clinical trials.

For decades ‘gene therapy’ has been a source of interest for clinicians and researchers, hope for patients, and sensationalism for the media. This talk will explore the path of the gene therapy revolution, from concept, bench, to bedside, and the ‘4^th hurdle’ of access to highly specialised therapies, after regulatory approval, using case examples, and personal experience.
